The proposed changes are an amendment to an existing guidance document. The existing guidance document describes eligibility for the Care Connection for Children (CCC) case management services and for the CCC Pool of Funds (POF). The Virginia Department of Health (VDH) reviewed this Guidance Document with stakeholders in November 2023. Stakeholders suggested minor changes to make the CCC POF guidelines clearer and easier to understand. The proposed changes specifically add language to clarify eligibility for the CCC case management services on page 2, remove language to reduce redundancies for the CCC POF on page 4, and clarify the process for reviewing client requests not covered by the POF guidelines on page 6. These proposed changes do not impact who is or who is not eligible for services or the POF. Additionally, minor changes were made for grammar and formatting.
