One time funds are HARD.  Surgery for chronic things like hernias and varicocoeles, joint replacements; treatment of chronic and curable infectious diseases are probably the most bang for the buck.  Possibly reno HVAC in FQHCs, Free Clinics?

Substance abuse treatment might be helpful.  Getting syringe services set up in every health department.   

Simplifying billing systems so that more therapists take medicaid?

Doing a pilot project temporarily raising medicaid reimbursement rates to medicare levels and seeing what happens?

Getting a state Pharmacy Benefit manager set up so you stop paying Aetna and welmark and caremark to make a profit out of being a pia with high cost drugs?
