Stating that exam must be completed within the last thirty (30) days by a licensed physician. 

This language is antiquated in our current healthcare landscape. Physician Assistants and Nurse Practitioners are commonplace in medical offices and often see patients and perform the physical exams. 

Recommendation:

Revise language to include "or designee".
