In regards to the Subject Policy above, the Sample Policy provided at pages 9-11 is well written and clear. I do have one recommendation on page 11, paragraph V, sub-paragraph E that addresses parents' being able to change their decision regarding the use of alternative instructional materials with notice to the schools, I recommend that notice use the same terminology as paragraph IV, sub-paragraph B on page 11 that requires notice to be given in writing by U.S. Mail, an e-mail, or verbally as part of a parent/teacher conference.
