| Purpose of the meeting The purpose of the meeting is to provide a public opportunity to learn about an innovative pilot in another state that focused on risk stratification and interventions for super-utilizers. In partnership with VCHI, DMAS has invited Dr. Tracy Johnson to present Denver Health's innovation study in a public forum for Virginia stakeholders. Specific topics covered will include: 1. Population stratification using risk corridors 2. Identifying clinically actionable real-time data 3. Operationalizing a different workforce to address the needs of the super-utilizer population | |
