The change proposed here is minor and makes little difference to this form; the form itself, is typical of the documents put into use by DBHDS to react to the demands of the DOJ Settlement Agreement.  An informal assessment and review by several knowledgeable families, individuals and providers resulted in comments that it is not useful and borders on offensive.
