As others said, the MPAI4 is for screening, not assessment. Has anyone at DMAS actually looked at what a comprehensive assessment based on the MPAI4 would look like? It would be very repetitive & they would all sound very similar, because for each rated item you say whether it is a mild, moderate, or severe problem based on the percent of time it interferes with activities.

So your assessment would be rather vague & sound like, "Bob has a mild problem with dizziness that interferes with activities 5 - 24% of the time. He has a moderate problem with motor speech (abnormal clearness or rate of speech, stuttering) 25 - 75% of the time" and so on if based on this tool.
