My Service Facilitator has become an intricate part of our family. This relationship began during a difficult time in my mother's health journey. My mother's health declined from a fall risk at 84 years old to a severe fall risk needing daily assistance while dealing with Alzheimer's disease at 87 years old. The Service Facilitator has eased my concerns, helped me navigate the paperwork, listened to my frustrations and basically gave a wealth of knowledge to help our family thru this process. The continuity of care is a guiding light in a tunnel with no end. You see my mother will remain a fall risk, she will progress in her Alzheimer's journey, and I will continue to give her care. I will also continue to look for the shining light and tether which is my Service Facilitator. She provides knowledge, support, paperwork, and an occasional reminder to breathe. We work as one unit to give the best outcome for my mother's health needs as well as her emotional needs. I can honestly say I have trusted my Service Facilitator with my mother's life. The information given by the Service Facilitator is not only accurate but supported by documentation that can be text, emailed or delivered to my home. That is what trust looks like. That is what commitment feels like. That is what quality and consistency of care should be. Please do not remove that lifeline from the care of so many families who depend on a Service Facilitator. Thank you.
