I am asking DMAS to carefully consider the impact these proposed changes will have on families like mine before making them final. Please preserve meaningful provider choice, continuity of care, and the individualized support families currently receive through Services Facilitation.
Any new model should not make it harder for families to navigate the system or take away relationships with trusted providers who already understand our needs. Families who depend on these services every day should have a meaningful voice in how these changes are made.
My mother is not just a Medicaid case or a number in a system. She is my mom, and decisions about these programs directly affect her quality of life and my ability to care for her safely at home.
Thank you for considering my family’s experience as you review these proposed changes.
Sincerely,
Tiara
