Guidance Document Change: These operational guidelines are intended to assist CSB DD case managers (support coordinators or SC/CM) in implementing enhanced case management requirements [12VAC30-122-190 A 6] at an increased level for individuals with more complex needs. This simplified version provides historical information, including updates from April 2014 and January 2017. It was developed by a committee of CSBs, state DD Waiver staff, and community advocacy representatives.

ECM for 90 days after a provider comes off a Provisional/Conditional License
 

If an individual is living in a home whose provisional or conditional license has been lifted, then the individual should no longer require ECM visits (unless he/she meets it in another area).  The home with a provisional/conditional license is under scrutiny by DBHDS Office of Licensure and they have to meet certain criteria for Licensure.  If/When the Office of Licensure makes the decision to lift the provisional or conditional license then the provider has been deemed suitable for an active license. The support coordinator will likely see that individual within 90 days after the license has been lifted and receive a person centered review from that provider and would not need to continue the 30 day visits.  If an individual moved into a  4 bed or less home after the license becomes active, that individual would not require ECM. It should state that once the license is no longer provisional/conditional then the individual would no longer require ECM.
