Description of Proposed Guidance Document Changes

DPOR has previously posted to Virginia Regulatory Town Hall many agency policies. This document is included in the agency’s listing of guidance documents. This document pertains to DPOR and its operations generally and is not regulatory in nature. This action will make amendments to the following guidance document on Virginia Regulatory Town Hall: Policy #300-01 – Regulant Name & Address Changes. The policy provides guidance to ensure consistent processing of name and address changes processed by DPOR. The following changes are made to the policy: a. Formatting of the document is changed. b. The policy statement is revised to provide that name and address change requests will be recorded in accordance with established licensing and enforcement systems data entry standards and Virginia law. c. Provisions for requesting a name or address change are revised to provide that requests may be completed using DPOR’s online licensing database. d. Provisions regarding requesting a name change are revised to clarify that (i) a divorce decree indicating a name change; and (ii) another state’s documentation that provides legal proof of name change are acceptable documentation of a name change. e. The policy is revised to provide that DPOR Examinations Section personnel who determine that a regulant’s name, main address, or physical address are different from those on record with DPOR must inform the regulant that a written request for the change with required documentation must be submitted to DPOR.
