Informed consent with responsible supervision
 

The thrust of my comments is (A) to ensure that owners must give fully informed consent whenever veterinary medicine students provide veterinary care, including the nature of supervision provided for each procedure or service to the owner's pet, and (B) to ensure planning, execution, and evaluation procedures that facilitate sound program design as a necessary ingredient for learning, for informed consent, and for mutual understanding by all parties. The latter is also intended to ensure that the decision-making process used by the veterinary school and licensed veterinarians supervising students is formalized, comprehensive, and supportive of the interests of all parties.  Specifically, I recommend inclusion of at least the following following types of provisions and stipulations, preceded by a clear statement of the program's mission, goals, values, and commitments::  (1)  Student-provided care, with informed owner consent, shall be provided only under the direct supervision of a licensed veterinarian for school-approved services and procedures, based on patient risk-level and student-considerations such as academic level, content mastery testing and skill-demonstration; (2)  After such general approval, the licensed veterinarian who accepts responsibility for the student's practice formally evaluates the information to concur that he or she is fully satisfied that the student can provide the specifically designated procedures safely and competently and that the nature of professional oversight required is within the scope of time and skills he or she can provide for the student to perform the various procedures; (3) Approval for the student to perform such procedures shall state whether the specified procedures require the licensed veterinarian to provide: (a) direct and continuous observation and supervision, or (b) immediate accessibility between veterinarian and student by telephone or other means of remote consultation and guidance while the student is performing the properly assigned veterinary procedure, and. (c) how back-up assistance will be provided timely in the event of critical unforeseen events during the student-performed procedure; (4)  Finallly, the school, which is responsible for ensuring proper implementation of all aspects of the design, shall periodicallly perform an outcome-based review, evaluation, and, as necessary, revision of its design and processes for student practice; such evaluation shall be conducted at intervals of three to five years and shall include feedback and suggestions from students, pet owners, participating licensed practitioners, and other relevant parties, such as school faculty not immediately involved in the practicum program and veterinary centers that have hired graduates of the school's programs. .
