The 'Service Selection Guide' pages are very helpful and should be included as part of the VIC document and not as a separate attachment.

The list of services on the 'written' VIC may cause confusion for some people if they do not know what services they have access to, but including the service guide would be helpful here.
There is no way to verify which provider options have been shared, question #8 does have the statement acknowledging that choice was provided but having them listed is helpful with verifying. The current electronic VIC for the RST requires that choices be listed out, and the form should maintain this.
