DMAS changes with CPST/Clubhouse International
 
  1. PSR appears to be a wrap around service with the new regulations. How will the other services like Case Management and Mental Health Skill building be utilized?
  2. I would like to recommend PSR programs have an extended time to be either CARF accredited or Clubhouse International accredited. 
  3. New Regulations state that members will not be able to have other services in place, such as ACT, CReST, or SUD. I am worried on the impact this will have on members who reside in group homes/ALF's and will no longer has structured days. Also, where will they get a chance to learn more about their mental health and social with others out in the community?
  4. The new rate is too low and PSR's will lose funding if we reduce the rate from $89.97(max of three units) to $72.41.
  5. The staff to client ratio 1 to 15. Our PSR is one of the larger ones in VA and will not be able to have low staff numbers to provide services. Our caseloads will be over this limit.
  6. Members enjoy participating in psychoeducational groups and this will no longer be acceptable. I recommend that we still incorporate groups for the educational piece in program. 
  7. Staff are no longer able to have offices with the Clubhouse Model. How will staff complete the necessary documentation for billing if they are required to be on the floor throughout the day?
  8. The new regulations are requiring evenings, weekends, and 3 cultural holidays. I fear this will create burnout with staff and make it difficult to maintain coverage. We have clients who are more symptomatic at baseline that require more monitoring, which at times requires more supervision.
