Chapter 3 Significant Changes - Identified location of commonly used forms such as the “service application” and “consent to exchange information” on the DARS public site and VDSS intranet; Clarified guidance on uploading signed service application to PeerPlace; Clarified guidance regarding short-term AS cases; Replaced “eligibility redetermination” with “ASAPS Financial Eligibility”; Clarified guidance for service plan updates of ongoing cases; Amended guidance related to transfers. Inserted clarification regarding transfer of ongoing APS registrations. Chapter 4 Significant Changes - Added guidance recommending annual HBC policy review; Clarified assistance with ADLs are part of companion activities; Inserted new section “Contracting for HBC services” and guidance; Clarified guidance on registering an adult in PeerPlace requiring an annual ALF reassessment; Clarified ePAS as the system of record for Medicaid LTSS and LDSS responsibility for documents related to Medicaid LTSS screenings; Added guidance on use of screening request log. Chapter 7 Significant Changes - Clarified guidance regarding LDSS obtaining qualification document; Updated location of form “Report of Guardian for an Incapacitated Person”; Added additional guidance on uploading guardian reports to PeerPlace.
