Where it states in 9.2 "the following information must be submitted with the preservice authorization request:...initial assessment including the completed CANS Lifetime," the implication is that there is more to the initial assessment than just the CANS.  Please clarify if more assessments are required, or simply state inclusion of the initial CANS Lifetime.
