Notice of Public Comment Period:

The Department of Medical Assistance Services (DMAS) and the Department of Behavioral Health and Developmental Services (DBHDS) welcome public comment regarding potential updates to the DMAS FORM #460 [Virginia Informed Choice Form].  The DMAS 460 is used across the Community Living, Family & Individual Supports, and Building Independence Waivers.  Drafts for review may be located at the following web addresses:
