Please clarify this:
"If the individual is unable to independently manage his/her own consumer-directed services, of if the individual is under 18 yares of age, he/she must an adult family member/caregiver or other chosen person serve as the employer or record (EOR)) on behalf of the individual. "

p. 97 Edit probably needed:

"Family and Independence Waiver"

p. 178 
"The state will also conduct targeted quarterly reviews at the participants home..... that they meet the needs of the individual."

Please identify which "state" agencies will be doing the reviews.
