Purpose of the meeting
The purpose of the meeting is to provide a public learning opportunity on strategies that improve health outcomes by addressing social determinants of health. We welcome Dr. Jeffrey Brenner, recognized for his groundbreaking work in helping to bring better care at lower cost for patients with complex health and social needs. The Senior Vice President of Integrated Health and Human Services at UnitedHealthcare, Dr. Brenner leads the myConnections business unit, UnitedHealthcare’s program to help low-income individuals and families access essential social services that are the gateway to better health. This series is supported through a partnership between the Virginia Department of Medical Assistance Services and the Virginia Center for Health Innovation. Space is limited and RSVP is required.
