| Purpose of the meeting Commonwealth of Virginia, Virginia Health Department (VDH), Notice of Intent to hold an advisory committee meeting and public hearing for the Preventive Health and Health Service (PHHS) Block Grant. All interested individuals and groups are invited to participate at the public hearing on the PHHS Block Grant. In accordance with Title XIX, Section 1905 of the Public Health Service Act, the Commonwealth of Virginia hereby gives notice that VDH will apply for FY 2017 PHHS Block Grant funds for programs addressing Healthy People 2020 national health objectives. The advisory committee meeting will be held on Monday, March 20, 2017 from 1:00 – 4:00 p.m. and the public hearing will immediately follow from 4:00 – 5:00 p.m. The committee meeting and public hearing will be held at the Department of Professional and Occupational Regulation, Perimeter Center Training Room 2, 9960 Mayland Drive, Henrico, VA 23233. The State Plan will be available on the agency’s website at www.vdh.virginia.gov/vdhlivewell/. Public comment on the plan can be made at the public hearing and written comments can be addressed to Robin Buskey, Office of Family Health Services, P.O.
