| Purpose of the meeting Commonwealth of Virginia, Virginia Department of Health (VDH), Notice of Intent to hold a public hearing and advisory committee meeting for the Fiscal Year 2016 Preventive Health and Health Service (PHHS) Block Grant. All interested individuals and groups are invited to participate at the public hearing on the PHHS Block Grant. In accordance with Title XIX, Section 1905 of the Public Health Service Act, the Commonwealth of Virginia hereby gives notice that VDH will apply for FY 2016 PHHS Block Grant funds for programs addressing Healthy People 2020 national health objectives. The public hearing will be held on Thursday, March 31, 2016 from 10:00 – 10:59 a.m. and the advisory committee meeting will immediately follow from 11:00 – 12:00 p.m. The hearing and committee meeting will be held at VDH, 109 Governor Street, Room 1023, Richmond, VA. The State Plan will be available on the agency’s website at www.vdh.virginia.gov/ofhs/. Public comment on the plan can be made at the public hearing and written comments can be addressed to Robin Buskey, Office of Family Health Services, P.O. Box 2448, Room 1021-J, Richmond, VA 23219. | |
